Putting the pieces together: how can IT help link up Primary Health, Acute Health, and Aged Care?

Many hospitals are now sending discharge summaries to GPs, but currently not much information comes back electronically the other way. Even less data makes its way into Aged Care systems.
We all know there is a lot of activity in developing and deploying new systems both across the Health sector and within Aged Care. However, currently much of this work is uncoordinated, creating the risk of new silos rather than an interconnected set of systems.
This session is designed to explore what information should transfer between the primary care setting, the acute setting, and aged care:
What data is clinically relevant across all sectors?
How could the movement/transfer of data between the sectors better support the patient journey?
What are the existing barriers to linking up systems in each sector?
What are some of the unique challenges that each sector faces in deploying systems?
What incentives exist or should be considered to encourage linking systems.
A panel of experts from Acute, Primary & Aged Care will explore these issues over a lunchtime session. There will be plenty of time for Q&A and for networking both before and after the event. Come along and contribute your views on this important topic.
