Even less data makes its way into Aged Care systems.\nWe all know there is a lot of activity in developing and deploying new systems both across the Health sector and within Aged Care. However, currently much of this work is uncoordinated, creating the risk of new silos rather than an interconnected set of systems.\nThis session is designed to explore what information should transfer between the primary care setting, the acute setting, and aged care:\nWhat data is clinically relevant across all sectors?\nHow could the movement/transfer of data between the sectors better support the patient journey?\nWhat are the existing barriers to linking up systems in each sector?\nWhat are some of the unique challenges that each sector faces in deploying systems?\nWhat incentives exist or should be considered to encourage linking systems.\nA panel of experts from Acute, Primary & Aged Care will explore these issues over a lunchtime session. There will be plenty of time for Q&A and for networking both before and after the event.
